Initial Visit

 

What to Expect

When you come to the office for your first visit you will be greeted by a smiling face welcoming you.  You will be acquainted with our facilities and ensure that you feel at home for this visit and for every visit to come.  Once you feel at home your first visit, the "Initial Consultation", will begin. 

Dr. Tammi will sit down with you and ask you some questions so that she can get a better sense of what you are seeking by coming to the office and how she can best assist you in reaching your goal.  She will then perform a series of assessments aimed at evaluating the health and integrity of your nervous system, spine and body, including a computerized posture analysis.  All of these findings are reflections of what is experienced within a person's life. 

At the end of the visit you will be scheduled for your follow up visit, the "Report of Findings" that we will schedule anywhere from 2 days to one week later.  During the "Report of Findings" Dr. Tammi will review your findings and make care and lifestyle recommendations for you based on how she feels she can best serve you, your needs and your goals. 

First Visit Forms

Please plan being in the office 40 minutes for the Initial Consult and 20 minutes for the Report of Findings.  Before you arrive for the Initial Consultation we ask that you have your History Form (0-14yrs, 14+yrs) completed and your NSA Consent Agreement read and signed.  We also ask that you read through our Privacy Notice and sign the Privacy Consent Form.  Please bring all completed paperwork with you to the first visit except for the Privacy Notice which is yours to keep.  We look forward to meeting with you and finding out how we can best serve you.
